We provide wound care to patients in their own homes, which is ideal for those who are unable to attend provider appointments easily, are bed-bound, or lack easy access to transportation.
We collaborate with other community providers, hospitals, and home health companies to ensure patients have access to wound care, regardless of their home status.
We also collaborate with several skilled nursing facilities and wound care providers serving patients in Reno, Fallon, and Elko. We would be happy to provide services to additional facilities seeking a wound practice to support their patient care.
With multiple providers and a combined experience of over 30 years in wound care, we are able to serve patients and their families in their homes, providing care for those who are unable to attend an in-office wound clinic.
We service patients once a week for home visits. We have team members on call from 8:00 AM to 4:30 PM, Monday through Thursday. Fridays, we are available by phone and see patients by appointment only.
We bill insurance for the cost of care, which is usually a covered service. If it is not, patients are informed upfront of their co-pay. We work with Medicare, Medicaid, and numerous other insurance plans to ensure patients receive the care they need.
Our providers receive comprehensive wound care training and regularly attend educational events to stay current on the latest advancements in wound care, enabling them to better serve our patients in this ever-evolving field. We provide care based on best practices and evidence-based treatment options to maximize the chances of a successful healing outcome.
We do not replace your existing home health services; we enhance them. Our providers collaborate closely with skilled nurses and physical therapists to support wound care plans through timely, specialized interventions. This collaborative model yields better outcomes while maintaining continuity of care across the broader team.
We provide diagnosis and treatment for a wide range of wounds, including but not limited to:
A referral is submitted by a provider or patient
Our team reviews insurance eligibility and confirms scheduling
A licensed provider visits the patient at home or in a facility
We develop a personalized care plan and share updates with referring teams
We continue regular visits and coordination based on wound status
Our team includes three full-time Nurse Practitioners, several per diem NPs, a wound care Registered Nurse, and several Medical Assistants who support our providers in the field. We have a combined experience of over 30 years in wound care across various settings, including acute, skilled, clinic, and home settings.

Tuesday – Fallon, Lovelock, Winnemucca, Battle Mountain
Wednesday – Elko, Reno/Sparks, Washoe Valley
Thursday – Gardnerville, Carson City, Dayton, Silver Springs, Yerington
Friday – by appointment only.
